This form should be used to cancel your appointment only. Please give us 72 hours’ advance notice of cancellation.
A scheduler will process the cancellation and send a confirmation email to you during regular business hours. This email is not for emergencies, urgent concerns or questions. If you have a question about your medical care, please contact us at (651) 209-1600.

Attention Cosmetic Patients:

Fraxel & Ulthera appointments cancelled and not rescheduled within 72 hours of the appointment will be charged a 15% service fee.
Sculptra appointments cancelled and not rescheduled seven days prior to the appointment will be charged a 100% service fee.
All other cosmetic appointments cancelled and not rescheduled within 24 hours of the appointment will be charged a $50 service fee.
